Since this book and all of my previous books came about through efforts toward excellence in teaching, I would like to dedicate this book to the colleagues in my teaching departments. The dedicated people in the Departments of Family Medicine and Obstetrics and Gynecology at Louisiana State University Health Sciences Center in Shreveport, LA, and the Departments of Family and Preventive Medicine and Obstetrics and Gynecology at the University of South Carolina School of Medicine in Columbia, SC, are outstanding examples of dedication to patient care and the art of teaching. These dedicated men and women exemplify what is best about both the medical and the teaching professions.
I thank my family, who taught me to see what could be, strive for things beyond myself, to do things that may make this world a better place.
