Over 20 countries were represented from many disciplines, and this eclectic mix ensured rich and varied debates on the problems facing health care in the twenty-first century.

In the pleasant environs of St Andrews, Scotland, scholars of health systems from across the world gathered in April 2002 to explore health care policy, system performance and issues of financing. From over 100 presentations at the Strategic Issues in Health Care Management (SIHCM) conference we have drawn this collection of papers as contributions to some enduring debates. While health care systems change with seeming ever-increasing rapidity, and problems appear perennial, we hope that you will agree that the extent, depth, subtlety and insightfulness of current health research offers policy makers and managers a better evidence-base than ever before to inform both strategic decisions and detailed actions.
