Preface
Since the first edition of this book in 1991 (which because of its popularity, went into a second printing in 1994) the principles outlined have become the standard of care worldwide. During the past 6 years, leukotriene antagonist tablets and new and better inhaled steroids have become available. Further developments in even safer and more effective treatments are on the horizon. Although it is evident that asthma is increasing in virtually all industrial societies, the cause of the increase, seen especially in children, remains obscure.
In the doses required by the vast majority of asthmatics to maintain almost continuous good health, inhaled steroids are safe and effective. The addition of long acting beta-agonist bronchodilators (symptom relievers) and the use of convenient and relatively small valved aerosol holding chambers to metered dose inhalers (puffers) or breath actuated dry powder inhalers ensures maximum benefit, ease of use and greater efficiency of administration, with virtual elimination of side effects and lowest possible cost of therapy. The greatest advantage of this approach is the promise of using a single, well understood safe treatment that empowers the patient to maintain good health over the long term by means of appropriate adjustments in the inhaled steroid dose, with an increase during rapid flare-ups and gradual reductions to a minimum maintenance dose during prolonged periods of freedom from symptoms.
