We are committed to the notion that our most vulnerable population should have access to the best and most current evidence-based care. From the moment we took on this project, this book has been a labor of love. The entire process has been punctuated with extreme highs and lows. Throughout it all, we enjoyed incredible support from our families, friends, and colleagues. Sadly, in 2013, we lost two of our most faithful and constant supporters: Kimberly Holmes (Lindas daughter) and Estelle Drozdz (Sandis mother). Our book is dedicated to their memory. These two amazing women never met, but both demonstrated remarkable strength in their lives. We are forever in their debt.

N o project this big can be accomplished alone. We are fortunate to have had the support of those closest to us as we worked to bring this book to completion. Deepest gratitude goes to our families for allowing us the time to work on this project. Wed also like to acknowledge the many professional colleagues whose work continues to inspire us and our elderly patients who have motivated us. Finally, we wish to thank Victor Van Beuren, the acquisitions editor from the American Diabetes Association for planting the seed for this book. Without his kind and gentle support over the past few years, this project would not have been possible.
