PROGRAM INTRODUCTION
A salutary revolution is taking place in the management of diabetes mellitus. Diverse pharmacological agents in novel classes are now available for favorable modification of both altered mediatary metabolism and insulin resistance, recognized to be pivotal in the pathophysiology of type 2 diabetes, the dominant form in the population. Accordingly, the publication of Medical Management of Diabetes Mellitus, the most recent entry in the Clinical Guides to Medical Management program, could not be more timely.
In this monograph edited by Drs. Leahy, Clark, and Cefalu, the field is reviewed in depth. It begins with the perspective of Doro Sims, a unique individual who has experienced the disorder and lived in the forefront of its elucidation as Dr. Ethan Sims' partner in life. The first part of the book, Overview, describes the impact of diabetes on our population, mechanisms contributing to its occurrence, standards of care and self-management, the nature of insulin resistance, and challenges confronting patients with diabetes in the managed care environment. In the second part, Signs, Symptoms, Diagnosis, and Diabetes Types, type 1 and type 2 diabetes mellitus are differentiated and described in detail as well as diabetes in pregnant women, in those afflicted with secondary forms or, with specific genetic syndromes giving rise to diabetes, and in children and adolescents. Part 3, Therapies, covers not only the key role of nutrition and exercise but also the use of oral pharmacological agents, conventional and novel insulin regimens, and the use of insulin pumps and pancreatic transplantation. Part 4, Complications, deals with complications encountered, often the features that bring the presence of diabetes to the attention of the patient and the physicial. Complications are described not only phenomenologically but also in terms of mechanisms responsible and responsivity to therapeutic interventions. The last part of the book, Special Settings, addresses diabetes in the elderly, obese subjects, hospitalized patients, diabetic patients undergoing surgical procedures, and women with a polycystic ovary syndrome who require special consideration.
Clinical Guides to Medical Management volumes are designed to bring a "consultant" to the elbow of the physician treating patients in the office, the clinic, the hospital, and emergency facilities. The concept underlying the series is that cardinal features of disease including prominent signs and symptoms must
