I wish to express my heartfelt and long overdue thanks to a number of organizations and individuals who have all contributed in some way to this book's creation. I began my research several years ago for a doctoral dissertation; the project was supported in large part by the generosity of the Claremont Graduate School (now Claremont Graduate University) through a Claremont Graduate School Fellowship and a Linhardt Dissertation Fellowship.
