Healthy Babies are Worth the Wait: Reducing Preterm Birth through the Integration of Primary Care and Public Health
The Challenge
In a report of births occurring in the US during the year 2006, it was stated, Preterm and low birthweight rates continued to rise . This pessimistic statement, suggesting a rise that could not be altered, was justified by more than two decades of steadily increasing rates of preterm birth or births less than 37 weeks of gestation. The preterm birth rate had risen by more than 20% from 1990 and 36% from the early 1980s. This increase in preterm birth was influenced not as much by births before 34 weeks, which rose modestly from 3.32% in 1990 to 3.66% in 2006, and more by late preterm birth (3436 weeks) which increased by 25% from 7.3% in 1990 to 9.15% in 2006. This same report noted that efforts to effectively prevent preterm birth were limited, even though over one-third of infant deaths were thought to be related to late preterm birth, and infants born late preterm had increased risks of long-term consequences.
Fortunately, not everyone considered the prevention of preterm birth an intractable problem, including the March of Dimes Foundation, which, supported by its Board of Trustees, declared preterm birth prevention to be a priority for the Foundation in 2003, ultimately revising the March of Dimes mission in 2005 to include the problem of premature birth in the US as a major focus and expanding this to a global campaign in 2008. Through the hard work of many organizations and individuals the preterm birth rate dropped in 2012 for the sixth straight year since the peak in 2006 to 11.5%, the lowest in a decade .
Notably, the Association of State and Territorial Health Officials (ASTHO) created the Healthy Babies Presidents Challenge of 2012 led by Dr David Lakey, Texas Commissioner of Health. This led to a committed pledge from the 50 states, Washington, DC, and Puerto Rico to reduce preterm births by 8% by 2014 .
Mobilizing an Integration of Primary Care and Public Health
The efforts of these individuals and organizations to reduce preterm birth have included programs involving the integration of primary care and , both in 2012.
The IOM report describes a continuum of integration between primary care and public health from isolation to merge with the goal being to achieve more efficacious improvement of the health of individuals, their communities, and the entire population. The intermediate steps along this continuum of integration include: mutual awarenessinforming each other of their activities; cooperationsharing resources between those in primary care and public health; collaborationplanning and executing with more intense and joint activity between these sectors; and partnershipintegrating the activities of the two sectors programmatically so that they appear inseparable. The report recognizes that initiatives may not be at the partnership or merger levels of integration at the outset, but their position within this continuum should not be a barrier to their initiation; the understanding of the steps along the way will provide a roadmap for improving integration in the process. The report boldly states, Beginning is more important than waiting until all of the requisite components are in place. Key principles in this process include strong leadership to foster integration across perceived disciplinary, programmatic, and jurisdictional boundaries; sustainability to maintain continuing success; and collaboration in the acquisition and analysis of data.
