Celiac disease is a syndrome characterized by damage of the small intestinal mucosa caused by the gliadin found in wheat gluten and other similar alcohol-soluble proteins, such as barley and rye, in genetically susceptible persons. The consumption of gluten in these persons leads to a self-perpetuating mucosal damage, while the elimination of gluten from the diet results in a full mucosal recovery. The clinical manifestations of celiac disease are protean in nature and vary markedly with the age of the patient, the duration and extent of disease, and the presence of symptoms occurring outside of the intestines. In addition to the classic gastrointestinal form, a variety of other clinical manifestations of celiac disease have been described, including atypical and asymptomatic forms. While in the past, celiac disease was typically considered a pediatric condition, we now are aware that the disease may become clinically evident after years of silent intestinal damage following exposure to gluten. Therefore, the diagnosis of celiac disease can be extremely challenging and currently relies on a sensitive and specific algorithm that allows the identification of different manifestations of the disease.Next page
