This book was written in two stages. It began life as a Glasgow University doctoral thesis researched and written between 2005 and 2010. It was then transformed into a monograph during 2012 and 2013. There are consequently a fair number of people who helped to shape it or supported me along the way. Firstly I would like to thank my PhD supervisors, Professors Colin Kidd and Dauvit Broun, for their expert advice and excellent suggestions, as well as their understanding of the challenges inherent to a part-time, distance-supervised PhD undertaken by a partially-sighted student.
