The idea for this book resulted from many discussions with Ken Plummer between 2009 and 2011. In the end, we did not end up writing it together. Nonetheless, many aspects of its structure and content are based on Kens ideas. I am grateful for these ideas and for a thoroughly enjoyable period of collaborative work.
