PREFACE
The field of melanoma research, like the proportions of the disease itself, is rapidly growing. Monographs dealing with melanoma have periodically been assembled, and over the years these have taken strongly epidemiological, pathological, surgical, or immunological perspectives. To date, however, few have approached basic and clinical investigations with a therapeutic multimodality perspective. This has been the rewarding challenge for the present volume, which is intended to bring together developments in areas that have currently achieved a meaningful therapeutic impact, or are soon anticipated to have major impact, upon the disease or its understanding at a basic level.
The contributors to this volume are leaders in their respective fields of investigation. They have each illuminated areas of controversy and added new insight to the basic molecular pathophysiology that bear on the diagnostic and therapeutic developments that will certainly continue to evolve in the next decade. Thus, the molecular investigation of melanoma progression is central to efforts that will someday yield molecular interventions capable of altering the onset

of this disease. The study of early melanoma and the immune response it engenders in situ will illuminate both the molecular pathology underlying the disease and the immunological response that is inextricably entwined with the outcome of this pathology at the host level. The discussions of sentinel node mapping and selective lymph node dissection, as well as radiotherapy and radiosurgery, for this disease reveal the substantial changes in therapeutic paradigms for melanoma that have occurred over the past few yearssince neither of these modalities had even been brought to systematic evaluation in melanoma until recently. The efforts to alter the course of the disease with chemotherapy and biologic therapy range from interferons to cytokines and now extend to vaccines able to induce consistent antibody response to defined gangliosides, and T-cell cytotoxic responses to protein and peptide antigens of melanoma. These agents have given clinical oncologists the tools to manipulate immune responses that are increasingly selective and to offer the prospect of improved cure and survival of this disease.
This volume is intended to introduce the major spheres of current melanoma investigation and to highlight the progress and issues that relate to melanoma biology, immunology, and therapeutics that presently occupy melanoma investigators worldwide. It is fitting that the volume begins and closes with considerations of the molecular biology and molecular pathology of the disease, including the primary and precursor atypical nevus, which will likely become a fruitful target of basic and therapeutic investigation research in the immediate future. We pay tribute to the earlier compilations in this field by Clark, Goldman, and Mastrangelo, Milton and McGovern, Ackerman, and Ferrone, but note that epidemiological, pathological and surgical, and basic immunological facets of the

disease have been the focus of works of the 1960s, '70s, and '80s. Therapeutic discussions have risen to the masthead only recently, and the degree to which they may figure into future similar volumes will doubtless continue to increase as the work described in this volume bears results.
