I dedicate this book to my wife, Gayle. I could not have completed it without her daily encouragement and dedication to our family. Her professional efforts to improve the lives of children and their families continually inspire me. I am very appreciative of my parents guidance and support. They have consistently demonstrated the value of working to help others while still making family a priority. I am very grateful for the support and encouragement of the many superb clinicians and neuroscientists with whom I have trained and worked. I particularly thank Harold Burton, Frances Jensen, Stephen Back, V. Leo Towle, Robert Clancy, Dennis Dlugos, Rebecca Ichord, Daniel Licht, Gihan Tennekoon, and Donald Younkin.

I am grateful for a full and gratifying professional life focused on pediatric critical care and anesthesiology. The dovetail of this clinical focus has been in research in the area of neuropathophysiology. I am indebted to a host of trainees, collaborators, mentors, students, and teachers. I am also indebted to non-physicians including nurses, nurse practitioners, respiratory therapists, pharmacists, and so many others, without whom the entire enterprise would collapse, and of course to my family; especially Michele, Sam, and Jon, without whom life would not be nearly as much fun and certainly not as rewarding.
