The following symptom survey will give a thorough picture of your health and any pain or discomfort you have, providing a great point of comparison for the improvements youll feel on the Aztec Diet.
SYMPTOMS QUESTIONNAIRE: The following questionnaire was developed for you to track improvements in your mood, sleep, energy, and physical symptoms on the Aztec Diet.
INSTRUCTIONS: Before you start the Aztec Diet, fill in the first column, marked Day 0. Every two weeks return to the table and enter how you feel.
SCORING: The scoring system is from zero to ten. Zero means the complete absence of that symptom. Ten indicates that the symptom is severe in its intensity. Mark your answer with a pencil in the columns below. If you have alarming symptoms such as shortness of breath with activity, tightness in the chest, or severe mood disturbances, review these with your doctor immediately. The most alarming symptoms are marked with an asterisk.
Hopefully you had some routine blood work done before you started the Aztec Diet. Compare your results with these tables of normal values to see what kind of progress you need to make; then have the labs repeated after youve made it to . You should see the numbers coming into line!
Cholesterol
These values should start to fall first in response to the Chia Challenge, in as little as five days. Large changes are seen after six weeks. These are the levels given by the American Heart Association.
