Beauty is a concept that has existed since the beginning of humanity. From Greek pottery to modern architecture, beautiful objects are celebrated as symbols of human achievement. In the same manner as we recognize beauty in art or nature, we also recognize it in the physical properties of humans. To insist on the irrelevance of beauty is to deny the importance of any other value that has developed in conjunction with humanity, including morality or coherence. The paradox of beauty is that although it is undoubtedly a significant human value, it receives undue emphasis from contemporary society. Furthermore, our conception of it has been monopolized and narrowed by mass media. What may once have been tangible, ideal human beauty has been removed from reality and transplanted (no pun intended) into the pages of magazines. We often strive for something that is almost a caricature of beauty rather than an appropriate goal.
People who undergo cosmetic surgery seem to be aware of the significance of beauty, but are they conscious of its overemphasis? We must first recognize the fallacy of modern beauty before we can achieve a realistic standard. Once we have recognized this distorted conception, then we can replace it with an undistorted one. With respect to beauty, this means discarding what we have been led to expect of it and establishing standards that are much less demanding and particular. Self-confidence is arguably the most important trait that a person can possess. It enables us to be more accepting of others and gives us emotional strength to question and act as individuals. Cosmetic surgery has the potential to be very beneficialit can make people look better and, as a corollary, feel more confidentbut only if our wants are sober. To view cosmetic surgery simply as the promotion of egotism and materialism is a fallacy. How can a mechanism that perpetuates a value that humanity has held since its origins, and increases self-confidence, have no worth? It must have worthit is illogical to think otherwise. Persons who have had, or are considering, cosmetic surgery are not necessarily vain or egotistical; instead, their decisions are often founded on a very humble desire: they want to feel better about themselves.

Preface
I can still recall phoning Dr. Norman Orentreich and asking him to review the first edition of this textbook before it was submitted for publication. An entire book on hair transplanting?!that was his inital reaction, before he graciously agreed to help. It was only sixteen years ago! Another nine years passed before I thought there was a need for a second edition, and yet less than five years later I became painfully aware that the second edition was no longer adequate. Improvements in equipment, anesthesia, minigrafting, alopecia reduction, scalp expansion and extension, and other areas, and the complexities in planning and organization that these changes produce in their wake, seemedand still seem to becoming faster and faster. Hence this new effort at providing an overview of all the various means of surgically correcting alopecia.
In this edition I have returned to the format of the first one, describing in detail my personal approach to each component of graft transplanting and, only then, discussing why I have chosen them over options used by other surgeons. This simplifies for the novice in transplanting an otherwise extremely complex menu of options. It provides readers with the information to mimic the approach of a single, well-experienced practioner in its entirety, or to substitute other approaches if they find someone else's technique perferable. I have tried to be fair in describing the options and have provided reasons when choosing one above the others. In addition I have included a lengthy chapter in which I have asked experienced and respected surgeons to elaborate on how their approach differs from mine, and whyMany roads lead to Rome. I have also asked others to write on areas of surgical hair restoration in which they were more knowledgeable than I. (Some of the chapters, or parts of them, have been left as they were in the second edition because the description is still current.) We are all fortunate that so many of our colleagues are so willing to share what they have leaned the hard way, so that we and our patients might be spared the problems they encountered.
