P erhaps no other health issue in the modern era has attracted as much controversy as HIV/AIDS, and perhaps no aspect of HIV/AIDS has been as contested as the question of how to prevent its spread. Indisputably, most prevention efforts have failed. In the first 25 years of the global pandemic, some 32 million people have died and another 33 million are currently infected. Although global HIV prevalence peaked more than a decade ago and is no longer rising, an estimated 2.7 million people are newly infected each year. In the countries of southern Africa and in high-risk subpopulations throughout the world, prevalence has stabilized at very high rates and the epidemic is far from over. A massive global effort has succeeded in putting approximately 5 million people on life-saving antiretroviral drugs (ARVs), but the international community seems to have reached the limit of its willingness to pay for such treatment, even as nearly 3 million more people are infected every
