This book, which grew out of my doctoral dissertation, was a long time in the making. Over the years I have run up a number of debts and this brief note is intended as a token of gratitude for all the people and institutions that have helped me along the way. I would like to thank the President and Fellows of New Hall, Cambridge for electing me to a Research Fellowship, the Leverhulme Trust for appointing me a Special Research Fellow, and Royal Holloway, University of London for acting as my host institution. The research I was able to undertake during this period proved invaluable.
