Preface

When I began my anesthesia practice, thoracic anesthesia was not considered an independent subspecialty. Today, 40 years later, it is one of the most challenging subspecialties, with innovations that include devices for lung isolation and monitoring capability, new understanding of the intraoperative managing of one lung ventilation, and a significant innovation in postoperative pain management. Over the years, thoracic workshops and thoracic symposiums have become integral during global anesthesia conferences. Thoracic anesthesia literature is now indispensable to many practicing clinicians libraries.

My intentions when editing this book were not to replace any thoracic anesthesia book, but rather to offer something slightly more comprehensive in the critical and rapidly expanding subspecialty of the anesthetic practice. I conceptualized this volume to serve as a true contribution to the field of thoracic anesthesia, appealing to clinicians who require the necessary skills and updated principles of thoracic anesthesia. The book is unique in that it redefines how we, as anesthesiologists, can achieve superior patient care. The perioperative care is discussed linearly, just as we encounter these situations in the hospital environment.

The book contains 55 chapters divided into six sections. The first section covers topics of pre-operative assessment including anatomic correlation of the respiratory system, radiology, and classifications of thoracic tumors from the surgeons perspective, as well as pulmonary pathophysiology and the role of hypoxic pulmonary vasoconstriction. The second and the third sections cover preoperative evaluation and preparation, monitoring of oxyanion and hemodynamics, and the role of bronchoscopy and high frequency ventilation during thoracic procedures. The fourth section describes the intraoperative management of one-lung ventilation, methods of lung separation, ventilatory strategies, fluid management, and pediatric patient care. The fifth section reviews the multimodal approach to acute and chronic pain management and postoperative complications including the use of extracorporeal ventilatory therapy. The sixth section contains a large number of specifics in clinical scenarios and case management such as video-assisted thoracoscopy, robotic surgery, tubeless procedures, and management of both geriatric and cardiac patients for lung resection. Other procedures include surgical and endoscopic lung volume reduction, esophageal surgery, cystic fibrosis, lung transplantation, tracheal resection, mediastinoscopy, tracheal stents, and enhanced recovery after surgery during thoracic surgery. Finally, the use of transesophageal echocardiography and ultrasound for thoracic procedures is discussed.
