Foreword
Host factors such as functional level and weight loss have long been known to have a significant impact on clinical outcomes for cancer patients. As early as 1932 research pointed out that malnutrition was a major factor contributing to mortality.
Since then, research offers a greater understanding of the changes in metabolic pathways leading to cancer cachexia (severe malnutrition and muscle wasting). Now scientists aim their research at uncovering the underlying mechanisms, such as production of so-called cytokines or humoral factors. These are chemicals, made either directly by the cancer or as a response by the patient's body to the cancer, which have many effects on the body's handling of food and calories, as well as appetite.
Some experimental evidence suggests that blocking or eliminating these factors will help reverse the cachectic state. This raises the hope of future intervention aimed at the root cause of cancer cachexia.
Another approach to malnutrition is through successful treatment of the cancer itself. This is known to reverse tumor cachexia. This has long been the focus of the medical profession, and we have seen much progress in many types of cancer treatment. As the authors pointed out, aggressive support is increasingly important to allow patients to complete aggressive treatment plans. Physicians and patients must understand that to maximize the benefit of the treatment, the cachexia must be addressed as part of the overall approach, and not as an afterthought.
