The diagnosis of hematopoietic neoplasms in the spleen and liver remains a challenge for many pathologists. Misinterpretation or misdiagnosis could occur due to an incomplete work-up in splenic or hepatic biopsies as well as in specimens post whole splenectomy. Biopsies are not usually obtained for primary diagnosis of hematopoietic neoplasms, as the diagnosis is most frequently made on other tissues, such as lymph nodes, peripheral blood, or bone marrows. Splenectomy is indicated mostly for therapeutic intervention rather than diagnostic purposes. Therefore, many practicing pathologists are challenged with the morphologic features and diagnostic approaches of hematopoietic disorders in the spleen and liver, especially of the rarer entities.
Following the emergence of modern diagnostic technology, e.g., next-generation sequencing (NGS), over the last decades, scientists have new insights into the pathogenesis of many hematopoietic disorders, benign or malignant. Many novel markers have been introduced to diagnosis, risk stratification, and prognostication. With the publication of the revised fourth edition ofWHO Classification of Tumours of Haematopoietic and Lymphoid Tissuesin 2017, we considered it would be timely to compile a new book on the current pathologic diagnosis of hematopoietic neoplasms in the spleen and liver. While a number of books have been published previously on this subject, significant progresses have been made on disease subclassification that requires integration of highly complex immunophenotypic and molecular features of a variety of common and rare hematopoietic neoplasms involving the spleen or liver. The objective of this book is to provide a comprehensive and most up-to-date overview of the pathologic features of common benign and malignant hematopoietic disorders in the spleen and liver for practicing pathologists, hematopathologists, and clinicians.
The authors are from large academic centers, affiliated teaching hospitals, and central referral clinics and are experienced in the diagnosis of hematopoietic disorders, not limited in the spleen and liver. In this book, we tried to cover the majority of diagnosing hematopoietic disorders in the spleen and liver. The book consists of 21 chapters. Chaptersencompass red blood cell disorders, other benign hematologic disorders, and infectious/inflammatory disorders that could mimic hematopoietic neoplasms. The histopathologic, cytogenetic, and molecular features of different disease entities are discussed in detail. To serve as a quick reference for pathologists, we have formatted most chapters on specific hematopoietic neoplasms to comprise epidemiology, etiology, pathogenesis, morphology, immunophenotyping, molecular genetics, prognosis, and brief treatment guidelines. The contents in the book are largely updated according to the most recent publications. Diagnostic caveats are included in the majority of book chapters in order to have a quick review of the key points in each chapter.
Compared with previous books, our book covers most, if not all, of the benign and malignant hematopoietic disorders in the spleen and liver. It is our sincere hope that general pathologists and hematopathologists will find this book practical and useful for their practice. The book would also benefit clinicians and researchers who are interested in hematology and hepatosplenic system.
