Introduction
One of the common sexual problems men face is premature ejaculation (PE). This happens when a man reaches his sexual peak within two minutes from the onset of sex with his partner. In extreme cases, climax usually occurs within 30 seconds of penetration.
When this happens, both parties are left feeling distressed and not fully satisfied; for the woman, her distress comes from her needs not being met, especially if this is a regular occurrence. Distress for the man comes from feelings of embarrassment and humiliation which he feels at his not-too-excellent performance, especially if his partner gets vocal about her displeasure.
About 30% of men worldwide go through quick ejaculation at one time or the other. A study by the National Health and Social Life Survey (NHSLS) places the percentage of men having premature ejaculation problems in the United States as 29%. Other studies report the following statistics:
- 30% of men in the UK
- 30.5% in Southeast Asian men
- 34% in African men
- 23.8% of men in Australia
In some cases, premature ejaculation is not too serious and the problem usually corrects itself when the man and sometimes with his partner, take some measures to correct it. However, a lot of men who experience premature ejaculation are usually too embarrassed to discuss the situation and would rather avoid sex altogether.
Premature ejaculation, most times, happens when the Pubococcygeus (P0) muscles located near the prostate gland lose elasticity and strength. These muscles are responsible for controlling semen expulsion in men and any loss in elasticity will result in an inability to control ejaculation.
Premature ejaculation can be classified into two:
- Lifelong or Primary premature ejaculation
- Acquired or secondary premature ejaculation
Lifelong Premature Ejaculation
This starts around puberty and usually occurs within one minute of sexual intercourse. In most cases, Lifelong premature ejaculation occurs when a man has conditioned himself to erupt quickly for one reason or the other. Most teens masturbate and for fear of getting caught and rush the activity to reach climax quickly. This becomes a habit with time that gets difficult to break.
Acquired Premature Ejaculation
Acquired premature ejaculation usually starts much later in life and could be as a result of a single factor or a combination of factors. Most times, the man would have had a normal sex life and would suddenly start having irregularities with his ejaculatory time.
Most acquired premature ejaculation problems are caused by issues that suddenly spring up in a mans life. On a broader scale though, both lifelong and acquired premature ejaculation problems could be caused by physical or psychological factors.
Causes of Premature Ejaculation
A lot of things could make a man suddenly develop ejaculatory problems. These problems are grouped into physical and psychological.
Physical Factors Causing Premature Ejaculation
- Diabetes:
Diabetes occurs when the body doesnt use insulin properly (type 2 diabetes) and is the most common type of diabetes, the other being a case where the body doesnt produce any insulin at all or doesnt use the insulin produced (type 1 diabetes).
Type 1 diabetes usually occurs in people before they turn 40 and is also called Insulin-dependent diabetes. This diabetes type happens when the beta cells that produce insulin in the pancreas get destroyed by the bodys immune system. The patients are then placed on insulin treatment and would need to eat healthy and exercise regularly to remain healthy. Only about 10% of diabetes cases are type 1.
About 90% of UK adults with diabetes are usually diagnosed with type 2, while it affects about 9.3% of US citizens. On a general scale, it affects about 7.3% of the world population. Symptoms of diabetes are usually frequent urination, increased thirst, fatigue, weight loss, blurred vision, slow healing of wounds and cuts, and irritability. With times, diabetes wears down the body, causing damage to the kidney, heart, nerves and eyes.
Prolonged diabetes damages some nerves in the body, resulting in erectile dysfunction. The pelvic nerves that connect to the penis get impaired and this affects blood flow to the penis. Nearly 50% of diabetic men suffer from erectile dysfunction, either as difficulties maintaining an erection or having premature ejaculation. Diabetic men also have a low amount of testosterone, which is one of the primary hormones responsible for sexual development.
- High Blood Pressure/ Hypertension
High blood pressure occurs when the force of the blood pushing against the blood vessels becomes too high.
The heart pumps blood to every part of the body with each heartbeat. When the rate at which this blood gets pumped gets too much, this puts the organ under some serious strain, resulting in stroke or heart attack.
There are no defined causes of hypertension in people, although it is believed that hereditary, obesity, smoking and alcohol, too much salt intake, all contribute to it. In most cases, hypertension occurs out of the blues, not showing any clear symptoms, which is why it is often referred to as the silent disease. In some cases though, symptoms such as nausea and vomiting, heart palpitations, prolonged headaches, heart palpitations and nosebleeds do occur.
Hypertension causes damages to sexual organs that control ejaculation, making it difficult to control.
- Thyroid Disorder
When the body produces an abnormally high level of thyroid hormones, it is said to be suffering from thyroid disorder or hyperthyroidism. The thyroid glands get over-active and produce more hormones than necessary.
The thyroid hormone is a very strong hormone and an abnormal amount in the body is strong enough to affect testosterone production in the testes, causing quick ejaculation. On the reverse, when the body produces an inadequate amount of thyroid hormone, it leads to hypothyroidism, which results in low libido and erectile dysfunction.
- Quick or Excessive Masturbation
Masturbation is a way of relieving sexual tension that builds up and is most times done solo. Most people who masturbate usually do so in private as some cultures and religion see this act as dirty and frown on it. This results in the man learning to stimulate himself in a way that he climaxes quickly before he gets caught. Overtime, the body gets conditioned to reach climax fast, and does so before the mans partner reaches her peak.
A few men over-masturbate; going as much as two or three times a day, several times a week. With this, the ejaculatory or PC muscles get weakened and are unable to hold on to the semen for long.
- Prostate Disease
Prostate diseases affect the prostate gland in men. These diseases range from prostatitis, prostate enlargement to prostate cancer.
The prostate gland conducts sperm from the production site to the penis. It is believed that 15% men get affected with prostatitis at some point in their lives. Prostatitis is a condition in which the prostate glands get inflamed and this affects how quickly sperm is released.
